2024-2025 “Keep-In-Touch” Enrollment Form

About the “KIT” Enrollment Form

You are invited to use this form to enroll your child in the Keep In Touch ministry.

Please complete the required fields and press the “Submit” button, at the bottom of the form. You will then receive a message that confirms successful submission of your enrollment.

Your child will then be included in our “Keep-In-Touch” Ministry.  We’ll be emailing notes of encouragement with e-Gift Cards in the months of:

September
November
February
April
