VBS Camp 2024 Registration Form
Registration for our 2024 VBS offering is now closed so that we can begin our final preparations. We are so sorry to have missed you this year, but we'll be back next year... hope to see you then!
About Our Enrollment Form
You may use this form to register your children in our Vacation Bible School Camp. Upon submission of this form, you will receive a message that confirms successful enrollment. If you have questions or need assistance, please contact the church office at (847) 359-1549. Registration closes at 11:59pm on Sunday, June 9
Cost
The cost of Vacation Bible School Camp program is $50 per enrolled child. As a courtesy to our volunteer camp “Leaders” and “Assistants,” a $10 discount per child is offered. These discount opportunities are automatically applied upon completion of the form.
All payments are non-refundable
Enrollment Instructions
All fields marked with an asterisk are required. If you have more than one child to register, be sure to answer “Yes” to the “Do you have another child to register?” question at the end of the form. Doing this will allow you to enroll another child. Once all of your children have been entered, and the answer to the above question becomes “No,” then you will be presented with the final questions and payment options. Safe and secure payment can be made with Paypal or any major credit card.
Completing Your Enrollment
Following payment and completion of all required fields, please click on the “Continue” button, located at the very bottom of the form. If you have questions or difficulties, please contact the church office at (847) 359-1549. For each child you have registered, you will immediately receive an email with our Photography and Medical Release document. Follow the directions to complete and return this required legal document. Your child is not registered until this form has been completed. Note that you will need your child’s Medical Doctor and Dentist’s name and phone number, plus your Medical Insurance Policy information and an emergency contact and their address/phone number.
Important Note: Please add the following two entries to your Contact List, so that you correctly receive the release form and any other emails from us regarding your enrollment(s). Otherwise, your email program may treat our email(s) as Junk or Spam and relegate them to your email’s program’s junk or spam folder. Adding the following to you Contacts list, prevents this from happening. If you don’t see our confirming email, please check your Junk and Spam folders.
